The investigation revealed a systemic pattern of safety violations at Voepass. The airline repeatedly reported false repairs and conducted operational tests to falsely certify faulty de-icing systems as functional, then continued operating aircraft with unresolved chronic defects by repeatedly renewing maintenance exemptions (MEL) without actual corrections. Between 2022 and 2024, the Brazilian aviation regulator (Anac) notified Voepass ten times for de-icing system irregularities; the airline claimed repairs had been completed, but the same faults recurred on subsequent flights. Cenipa investigators identified a "culture of normalizing deviations" within the company. The aircraft that crashed had experienced the same de-icing failure on three consecutive flights before the accident.
During the fatal flight, the aircraft received eight icing warnings while cruising. The captain stated he had activated the de-icing system, but recorder data indicated it remained off. The crew did not declare an emergency despite receiving critical warnings about performance degradation, a phenomenon Cenipa termed "inattentional blindness and deafness." The aircraft lost lift, entered an unrecoverable flat spin, and crashed into a residential neighborhood in Vinhedo, killing 58 passengers and four crew members.
Cenipa also identified contributory failures by Brazil's aviation regulator. Anac's oversight allowed Voepass to continue operating despite documented violations. In a subsequent interview, Anac Director Álvaro Faierstein stated the agency had been deceived by Voepass, which submitted falsified maintenance documents showing completed repairs that had never actually been performed. He noted that in 2023, Voepass accounted for 82 percent of all maintenance violations cited by Anac across Brazilian airlines while operating only 0.6 percent of the nation's flights. Anac permanently suspended Voepass operations in June 2025, ten months after the accident.
Victims' families have announced plans to file a class-action lawsuit for moral damages against Voepass, Anac, aircraft manufacturer ATR, and individuals who may be charged by federal police. The Cenipa report, characterized as having investigative rather than prosecutorial intent, is expected to inform ongoing federal police criminal investigations into potential charges related to the crash.
