XFG Covid Variant Drives UK Hospital Admissions Up 400% in Month

The UK Health Security Agency (UKHSA) has reported a sharp rise in Covid-19 cases and hospitalizations linked to the XFG variant, also known as Stratus or "American Covid," as autumn approaches. Monthly figures show cases jumped 230 percent from 1,374 in August to 4,589 in September, while hospital admissions rose approximately 420 percent from 950 to 4,939 over the same period. In a single week to late September, cases increased by nearly 30 percent. The variant now accounts for more than half of all sequenced positive samples in England, with the rate of positive Covid tests in hospitals rising from 5.9 percent to 7 percent.
London has experienced the most significant surge, with hospital admissions up by approximately 79 percent. Despite the steep rise, officials emphasize that current hospitalization levels remain far below pandemic peaks—January 2021 saw over 20,000 people hospitalized. The XFG variant is a recombination of earlier Omicron lineages (LF.7 and LP.8.1.2) that was first identified in January 2025 and has since spread globally. Dr. Alex Allen, a consultant epidemiologist at the UKHSA, stated that XFG is "closely related" to strains circulating for over a year and does not pose a greater threat than previous variants.
Symptoms of XFG remain largely consistent with earlier Covid strains, including sore throat, cough, fever, fatigue, headache, and muscle aches. Medical professionals have noted that hoarseness and a scratchy throat appear more common with this variant compared to earlier strains, though these symptoms are not unique to XFG. The World Health Organization is monitoring the variant but has not designated it a variant of concern. Experts attribute the recent surge to waning immunity from prior infections and vaccinations, increased indoor contact during colder months, and the variant's current dominance in circulation.
Health authorities have advised eligible people to receive updated Covid and flu vaccinations, with this year's autumn vaccine specifically formulated to protect against XFG and related variants. The NHS guidance remains unchanged: those over 18 who test positive should isolate for five days, while those under 18 should isolate for three days. Close contact with immunocompromised individuals should be avoided for 10 days following a positive test. Most cases remain mild, and treatment typically involves rest, fluids, and over-the-counter fever or pain relief medication, though antivirals such as Paxlovid may be prescribed for higher-risk individuals.
